Нейропротективные свойства мемантина при церебральной ишемии [131, 132]
Нейропротективные свойства мемантина подтверждены в эксперименте: на фоне введения мемантина выраженность ишемических повреждений уменьшалась в среднем на 25–36% [131, 133]. Получены данные, свидетельствующие о способности мемантина уменьшать выраженность не только ишемических, но и реперфузионных повреждений нейронов [134].
Эффективность мемантина в условиях экспериментальной ишемии
Препарат обычно хорошо переносится больными, лишь в единичных случаях могут возникнуть двигательное беспокойство, нарушения сна и головокружение [135, 136]. Побочные реакции чаще всего обусловлены слишком быстрым увеличением суточной дозы препарата. Рекомендуется постепенно, в течение 3–4 нед увеличивать дозу мемантина от 5 до 20 мг/сут [135]. Учитывая хорошую переносимость и безопасность мемантина, лечение может быть весьма продолжительным (до 1 года). Определенное значение при ведении больных с легкими когнитивными нарушениями придается разрешению стрессовых ситуаций, если они есть [37]. Это связано не только с возникновением на фоне стресса депрессии и тревожности, проявляющихся также снижением памяти, но и с тем, что на фоне хронического стресса гормоны коры надпочечников неблагоприятно влияют на гиппокамп [37]. Еще одним направлением в ведении данной категории больных является умеренная физическая активность, которая благоприятно влияет на когнитивную сферу [37]. Используется также диета с низким содержанием жиров и с повышенным – антиоксидантов [37]. Повышенным содержанием антиоксидантов отличаются ягоды (черника, земляника) и томаты.
